Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. The role includes visiting members in their homes, reviewing medical histories, performing physical evaluations, and coordinating with health plans. You will travel locally and within licensed states, and may conduct virtual visits as needed. Qualified candidates hold an active, unrestricted NP or PA license, board certification, and are #J-18808-Ljbffr
