Registered Nurse - Habilitation Nurse

Contract Nurse Position

We are seeking contract nurses to support the increasing nursing caseloads across the state. The position's location will be determined after candidate selection, with a focus on the District South Area. Candidates should be located in this area.

We aim to add five contract nurses to our existing team of approximately 35 full-time nurses, supporting around 1,800 members with skilled nursing needs. These contract nurses will be utilized differently than our current nurses. They will serve as floaters, stepping in during absences such as annual leave, sick leave, or FMLA.

Key Functions

• Location varies required for multiple districts.
• Working hours: 8:30 am - 5:00 pm.
• FPCC needed.
• Travel reimbursement and overtime may be required.

Description of Duties

• Conduct nursing assessments and collaborate with external agencies to ensure appropriate services are delivered.
• Authorize skilled nursing services for members through assessments.
• Conduct in-person visits statewide, serving a core district, monitoring health needs, assisting with referrals, and advocating for members' rights.
• Complete reports documenting the medical necessity for skilled services as required.
• Perform in-person assessments every 90 days and as needed. The position is field-based, requiring travel.
• Complete nursing referrals and vendor calls.
• Handle special projects as needed.
• Complete documentation within established timeframes.
• Participate in interprofessional collaboration and multidisciplinary planning meetings.
• Engage with members and their support systems.
• Attend mandatory meetings and complete required annual trainings.

Experience and Expertise

• Registered Nurse licensed to practice, active, and in good standing without restrictions.
• Additional training or certification in behavioral health, complex medical conditions, or nursing case management is preferred.
• Minimum of two years of recent RN experience in home health, hospice, long-term care, or case management.
• Excellent communication skills, with the ability to explain complex information to members and their support systems.
• Proficiency with Google Office, Microsoft Office, and Adobe Acrobat, including instant messaging, document uploading, and a willingness to learn new software for state documentation.
