Home Health Physical Therapist Assistant (PTA) – Nashville, TN – Full Time All Heart Home Care is a health‑care company dedicated to improving the quality of life for patients in their own homes. Position Overview We are seeking a qualified Physical Therapist Assistant to treat patients of all diagnoses to relieve pain, restore function, and maintain maximum performance using exercises and modalities. The goal is to help patients recover faster, regain independence, and prevent rehospitalizations. Responsibilities Develop and implement individualized treatment plans for home‑health patients. Provide therapeutic interventions including exercise, manual therapy, and modalities. Educate patients and families on self‑care techniques and safety measures. Document progress and update care plans in accordance with regulatory requirements. Collaborate with supervising physical therapists, physicians, and other health‑care professionals. Qualifications Current, valid, and unencumbered Tennessee PTA license. Current CPR certification (or obtain within 30 days of hire). Valid driver’s license with reliable transportation and auto insurance. One year or more clinical experience preferred.
