Nurse PractitionerTryfacta is seeking a Nurse Practitioner for our client in Oxford, NY 13830. This is a 2-year contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!Location: Oxford, NY 13830Duration: 2 yearsWork Schedule: 8:00 AM and 4:30 PMResponsibilities for this position include, but are not limited to:Conduct routine and as-needed resident assessments, examinations, and follow-up care.Develop, monitor, and adjust treatment plans in coordination with the interdisciplinary care team.Document all resident encounters, orders, and clinical findings in the electronic medical record (EMR) system.Participate in care planning meetings and collaborate with nursing leadership, physicians, and other healthcare professionals.Provide onsite and on-call clinical support while ensuring continuity of care and timely medical response.To be considered for this position, you should have:Active New York State Nurse Practitioner (NP) license or Physician Assistant (PA) license with current certification.Minimum experience in long-term care, skilled nursing facilities, geriatrics, rehabilitation, or related healthcare settings.Strong clinical assessment, diagnostic, treatment planning, and patient management skills.Proficiency in electronic medical records (EMR) documentation and interdisciplinary care coordination.Excellent communication, teamwork, and ability to provide responsive onsite and 24/7 on-call coverage.Tryfacta is an Equal Opportunity/Affirmative Action Employer.
