Physician Assistant (PA)

for an outpatient primary care opportunity in

Comprehensive primary care across the lifespan Diagnosis and management of acute and chronic illnesses Preventive care and wellness visits Chronic disease management Community outreach and continuity of care Team-based care model Modern outpatient practice environment Key Responsibilities

Perform comprehensive patient evaluations, physical examinations, and health assessments Diagnose and treat acute and chronic medical conditions Develop individualized treatment and preventive care plans Order, interpret, and communicate laboratory and diagnostic test results Prescribe medications and other therapeutic interventions in accordance with state regulations Educate patients on disease prevention, healthy lifestyle choices, and chronic disease management Collaborate with physicians, nurses, behavioral health providers, and community care teams to coordinate comprehensive patient care Participate in occasional community-based patient visits to support continuity of care Maintain accurate and timely electronic medical records Support quality improvement initiatives and organizational goals Qualifications Required

Graduate of an accredited Nurse Practitioner or Physician Assistant program Current Massachusetts NP or PA license (or ability to obtain) Certification as one of the following:

Family Nurse Practitioner (FNP) Adult-Gerontology Nurse Practitioner (AGNP) Physician Assistant-Certified (PA-C)

DEA registration (or ability to obtain) Excellent communication and interpersonal skills Commitment to patient-centered, team-based care Preferred

Minimum of

one year of primary care clinical experience Experience managing patients with multiple chronic conditions Bilingual language skills are a plus Experience working in community health or value-based care settings Compensation & Benefits
