What you'll do: Serve as the primary Hospice RN Case Manager for your assigned patients and families. Complete comprehensive nursing assessments and develop individualized plans of care. Manage pain and symptoms while promoting comfort, dignity, and quality of life. Educate patients, caregivers, and families throughout the hospice journey. Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and community partners. Respond to changing patient needs while providing compassionate guidance and clinical expertise. Maintain accurate, timely EMR documentation while ensuring compliance with Medicare hospice regulations.

What you'll bring: Active Minnesota Registered Nurse (RN) license. Two or more years of hospice experience preferred, or a strong background in oncology, palliative care, home health, ICU, or other complex care settings. Excellent assessment skills and independent clinical judgment. Strong communication and relationship-building abilities. Compassionate, patient-centered approach to end-of-life care. Exceptional organization, time management, and critical thinking skills. Reliable transportation and willingness to travel throughout your assigned territory.
