Your Role

As a PRN Home Health Speech Therapist , you’ll evaluate and treat patients experiencing communication, cognitive, speech, language, and swallowing challenges related to illness, injury, surgery, or other medical conditions.

Key Responsibilities

• Perform comprehensive speech, language, cognitive, communication, and swallowing evaluations in patients’ homes

• Develop individualized plans of care based on each patient’s needs and goals

• Provide skilled speech therapy interventions to improve communication, cognition, and swallowing function

• Educate patients, families, and caregivers on therapeutic techniques, safety, and strategies for continued progress

• Monitor patient outcomes and modify treatment plans as appropriate

• Collaborate with physicians, nurses, physical therapists, occupational therapists, and other members of the interdisciplinary care team

• Maintain timely, accurate, and compliant clinical documentation

• Help patients maximize independence, safety, and quality of life at home

What We’re Looking For

• Current Speech-Language Pathologist (SLP) license in Arizona

• Current CPR certification

• Strong clinical assessment and critical-thinking skills

• Excellent communication and interpersonal skills

• Ability to establish trust and rapport with patients and families

• Ability to work independently in a home health setting

• Strong organizational and time-management skills

• Valid driver’s license and reliable transportation

• Commitment to compassionate, patient-centered care

Home health experience is preferred, but we welcome qualified Speech Therapists from other clinical backgrounds who are interested in transitioning into home health.
