Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high-risk members.
• Collaborate with primary care providers for care plan implementation.
• Address and reassess cost-efficient, appropriate levels of care.
• Coordinate with healthcare professionals for patient care transition.
• Engage specialty resources as needed for optimal outcomes.
• Maintain detailed records of clinical, functional, and fiscal outcomes.
• Identify opportunities for health promotion and illness prevention.
• Act as patient advocate, ensuring privacy and confidentiality.

Requirements of the RN - Case Management

• GA RN License; compact license accepted
• Associates or Bachelor's Degree in Nursing or related field
• Case Management certification
• Experience in wound care, IV care, HCHB, and home health
• Ability to analyze complex medical information and make decisions
