License/Certifications: Valid Physical Therapist license for Oklahoma required

Experience: Previous experience in Long Term Care preferred

Hourly Pay Rate: $56.95

What You'll Do

• Screen, evaluate, and develop individualized care plans for patients with physical functioning disorders or wound care needs
• Administer rehabilitative procedures including manual techniques, therapeutic exercises, modalities, and use of supportive devices
• Assess patient progress and modify treatments based on outcomes and referrals
• Document clinical findings, treatment minutes, and discharge summaries accurately in electronic health records
• Collaborate with the healthcare team regarding patient progress, barriers, and adjustments to care plans
• Supervise and support physical therapist assistants, rehabilitation aides, or students when necessary
• Participate in care conferences, training, and facility meetings as required

What You Need

• Valid Physical Therapist license in Oklahoma
• Prior experience in long-term or pediatric care settings preferred
• Strong communication, assessment, and documentation skills
• Ability to work independently and as part of a multidisciplinary team
