Job Description & Requirements Specialty: Hospice Discipline: RN Duration: 13 weeks 40 hours per week Shift: 8 hours Employment Type: Travel Hospice RN | Missoula, MT | 13 Weeks Location: Missoula, Montana Specialty: Hospice RN / Case Manager Contract Length: 13 Weeks Schedule: Monday–Friday | 8:00 AM–5:00 PM ⏰ Guaranteed Hours: 40 Hours/Week EMR: Homecare Homebase (HCHB) ⚡ Position: Rapid Response Crisis Position Overview We are seeking an experienced Hospice RN / Case Manager to provide skilled nursing, hospice care, and case management to patients in their places of residence. The RN will admit patients to services, coordinate care and resources, provide patient and family education, and work collaboratively with an interdisciplinary team to promote quality outcomes and quality of life. This position may initially begin as a Visiting RN and transition to a Case Manager. The same clinical responsibilities apply to both roles. Responsibilities Provide skilled nursing care to patients and families in residential settings. Assess, observe, and evaluate patients' physical and psychosocial conditions. Develop and implement individualized plans of care. Monitor changes in condition and responses to treatments and medications. Oversee care provided by Home Health Aides and CNAs. Coordinate care with other disciplines, community health agencies, support services, and social agencies. Provide symptom management, teaching, counseling, interventions, and referrals. Maintain accurate, current, and complete patient records in accordance with agency standards and OASIS requirements. Participate in team scheduling and on-call rotation, including telephone assessment and home visits when required. Participate in quality improvement activities and required in-service training. Perform other duties as assigned by the supervisor. Requirements Minimum 3 years of recent acute care, home care, or hospice experience Current, active Montana RN license Valid driver's license Appropriate auto insurance Reliable transportation Strong clinical assessment and care-management skills Experience with clinical skills such as: Venipuncture Catheterization Wound care Parenteral therapy HCHB experience strongly preferred BSN preferred CloudinfraIT Inc Job ID 299446.
