The remaining four days will be spent providing care in a sub-acute nursing home setting. The position includes on-call responsibilities every fifth week, covering after-hours of 5:00 PM to 9:00 AM, Monday through Friday, and all day on weekends.Requirements:APN LicenseRN LicenseNP CertificationBLS and ACLSCDS and DEASchedule:Full TimeDays1 day in the office and 4 days at external facilitiesEssential Functions:Rounds regularly at one to two subacute care facilitiesPerforms transitional care visits for patients discharged from subacute/nursing home facilitiesConducts home visits as part of ongoing patient carePreferred background or strong interest in palliative careCompletes POLST (Practitioner Orders for Life-Sustaining Treatment) formsCollaborates closely with physicians within the practice to coordinate and align patient carePerforms comprehensive initial and episodic health assessments; in order to identify existing or potential health problemsInitiates and implements an individualized nursing and medical plan of care in collaboration with members of interdisciplinary teamInitiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with physicianProvides follow-up care including acute and routine examinationsAssesses progress, identify problems, initiate preventive measures, recognize emergent problems, initiate proper care in collaboration with physicianMonitors and adjusts patients medication according to protocol or medical plan of careImplements nutritional therapies, as appropriateTriages patient problem calls which requires medical attentionOrders appropriate laboratory and diagnostic proceduresRecognizes normal and abnormal laboratory findings and the pathology involvedDocuments information in patients charts following documentation standardsProvides health education to the patient and family (e.g., management of side effects, anticipatory teachings)Manages medical emergencies as indicated, and according to standing ordersProvides monitoring and continuity of care between health care settingsProvides patient and family counseling and reassurance in regard to psychological and or psychosocial issuesInitiates proper referrals to private or community agenciesActs as a resource and teacher for other members of the health care teamShares knowledge and expertise with staffCollaborates with the nursing and medical staff at partner and affiliate sites to coordinate continuity of careEvaluates the quality of care as well as the quality of life for patients through assessment of patient response and recommend interventions, as indicated
