Northstar Behavioral Health is seeking a compassionate and detail-oriented MAP Care Coordinator to provide essential case management services to women in Ohio facing substance abuse challenges – many of whom are pregnant or living with persistent mental health conditions. Ensure their access to local resources and assist members in applying for benefits and health insurance such as SSI, SSDI, Medicaid, or Medicare.Actively reach out to clients who have not been in contact to ensure engagement in services in their home or transitional housing placement.Outreach and engage potential clients to evaluate appropriateness and desire for services in jails, hospitals, or street settings.Provide needed therapeutic interventions and conduct support groups to help clients improve their level of functioning.NorthStar Behavioral Health is an Equal Employment Opportunity (EEO) Employer.
