Position Summary

The Speech Language Pathologist (SLP) evaluates and treats residents with speech, language, cognitive, voice, and swallowing disorders. This role collaborates closely with nursing, therapy, and medical teams to support safe communication and nutrition outcomes in a skilled nursing environment.

Primary Responsibilities

Evaluation & Treatment

• Evaluate residents for speech, language, cognitive, and swallowing disorders

• Develop and implement individualized treatment plans

• Provide therapy to improve communication, cognition, and safe swallowing

Documentation & Compliance

• Document evaluations, progress notes, and discharge summaries accurately

• Ensure compliance with state, federal, and facility documentation standards

Interdisciplinary Collaboration

• Communicate treatment plans and progress with nursing, rehab, and medical staff

• Participate in care plan meetings and interdisciplinary discussions

• Educate residents, families, and staff as appropriate

Qualifications

• Master’s degree in Speech-Language Pathology

• Current NYS license as a Speech Language Pathologist

• ASHA CCC-SLP preferred

• Experience in skilled nursing or long-term care preferred

• Strong clinical, communication, and organizational skills
