Dementia Care Coordinator

Great Lakes Inter-Tribal Council is a non-profit organization formed by the Native American Tribes of Wisconsin. We serve all WI tribes by providing administration of grant-funded programs either directly or with sub grants. Programs serve a range of functions from Children to Elders. Our Mission is: To Enhance the quality of life for all Native People.
Dementia Care Program Coordinator
Responsibilities include assisting with the planning, implementation, and evaluation of the assigned Dementia Program workplan; project management; and staff management. The Program Coordinator will assist with overall implementation of the Dementia Care grant, including providing training to Tribal communities in the Bemidji area. Working as a member of the program team, this role will use advanced analytical, research, project management, and relationship management skills to deliver on priority work plan activities, including program assessment, planning, implementation, and evaluation. Frequent travel is required to meet the needs of the position. This position is in our Green Bay, Wisconsin office. The Dementia Care Program Coordinator has a primary duty to assist the Dementia Care Program Director with day-to-day operations, program scheduling, data organization, reporting, and partner communications. Additionally, the program coordinator co-creates and refines new program offerings and provides "Train the Trainer" training and dementia education. Principal Duties and Responsibilities:
• Conducts program duties necessary for funding, which include grant writing, compliance, and reporting. • Conduct outreach with tribal communities. • Dementia education, presentations, and training as needed and requested. • Organizes meetings and community listening sessions. • Preparation of reports for Tribal Nations and funders. • Other duties as assigned by the Program Director Working Relationships and Scope:
Reports directly to the Dementia Care Program Director. In regular contact with GLITEC and GLITC staff. In communication as appropriate with Tribal Health staff, cultural practitioners, and Tribal community partners.
Performance Dimensions:
Supports the mission of Great Lakes Inter-Tribal Council (GLITC). Demonstrates a basic understanding of tribal customs, traditions, values, and beliefs. Quality, quantity, accuracy, timeliness and thoroughness of work performed; quality of communications and development of good working relationships with others internal and external to the Agency; honesty and integrity in the execution of all activities; customer satisfaction with services provided; maintain and comply with all Agency policies and procedures; reliability in reporting to work regularly and on time; soundness of business decisions made; maintain the integrity of confidential business information; stay current in field and utilize that information where practical. Demonstrate a friendly and helpful approach and attitude to internal and external customers; professionalism; and commitment to GLITC's Mission, to enhance the quality of life for all Native people. Knowledge, Skills, and Abilities:
Bachelor of Public Health, Bachelor of Science, or bachelor's in healthcare administration, with two years' project management experience and or training. Experience working with elder care, Alzheimer's Disease, or dementia-related public health programs preferred. Demonstrated understanding of cultural, social, and systemic factors affecting elders in Tribal communities. Understanding of traditional American Indian health issues. Experience working with Native American Tribal Governments, Tribal organizations, and/or not-for-profit agencies in epidemiology. Ability to work independently and as a program team member. Individuals must possess excellent verbal and written communication skills; professional demeanor and appearance; problem-solving skills; ability to manage time effectively; self-motivation skills; work effectively under pressure; ability to build positive work relationships with internal and external customers; ability to interact professionally with a wide variety of people; and speak in front of groups. Individuals must be able to address a wide range of intellectual and practical problems, apply statistical calculations to findings, and compose complex business correspondence (reports, health profiles). Requires a high level of confidentiality. You must possess a valid driver's license and/or reliable transportation with adequate insurance and the ability to travel. This is a full-time exempt opportunity working Monday through Friday, in our Green Bay, Wisconsin office. This is not a remote position. The salary range for this position is $55,000 - $61,000 annually, dependent upon education and experience, with benefits including: Annual/Personal Leave and Holiday Pay; Health, Dental, Life & AD&D, STD and LTD Insurance, FSA and 401(k) Retirement Plan.
