BioMend Health Solutions PLLC is a clinician-led healthcare practice delivering compassionate, high-quality Mobile Primary Care and Advanced Wound Care directly to patients in their homes and residential care facilities across Washington State. We are seeking dedicated, compassionate Nurse Practitioners (ARNPs) and Physician Assistants (PAs) based in Vancouver, Washington to join our growing mobile clinical team. In this role, you will provide comprehensive primary healthcare and specialized advanced wound care treatments to patients in mobile and community settings. Key Responsibilities:

• Primary Care Management: Perform routine health evaluations, chronic disease management, preventive screenings, medication management, diagnostic ordering (labs/imaging), and urgent care consultations.
• Advanced Wound Care Procedures: Conduct comprehensive wound assessments, staging, and therapeutic interventions, including sharp debridement, skin substitute/amniotic membrane applications, compression therapies, and non-contact ultrasound (Ultramist®) therapies.
• Patient & Family Education: Educate patients, families, and caregivers on care plans, wound prevention, disease management, and post-procedure protocols.
• EHR Documentation: Maintain timely, accurate, and complete clinical documentation in the company’s EHR system in accordance with regulatory and billing guidelines.
• Care Coordination: Collaborate closely with clinical leadership, medical assistants, patient liaisons, and interdisciplinary healthcare teams to ensure seamless continuity of care.
• Travel & Home Visits: Conduct in-person visits to patients' homes and residential facilities within assigned geographic coverage areas.

Requirements:

• Education: Master’s or Doctorate degree in Nursing (MSN/DNP) or Physician Assistant Studies from an accredited institution.
• Licensure: Active, unrestricted Washington State ARNP or PA license in good standing.
• Certifications:
  • Board Certification (e.g., FNP, AGNP, PA-C).
  • Active DEA License (or eligible to obtain).
  • Current BLS and ACLS certifications.
  • Credentialed with Medicare, Medicaid, and major commercial payers (or eligible for credentialing).
• Transportation: Valid driver’s license, reliable personal vehicle, and active automobile insurance.
• Skills & Attributes: Strong clinical acumen, excellent communication skills, ability to work independently in field settings, and a patient-centered approach to care.
