Responsibilities

• Plan and conduct PT evaluations to inform individualized treatment plans
• Provide therapy interventions using established protocols and techniques
• Collaborate with the interdisciplinary team and participate as a co-leader in patient groups
• Develop and communicate patient care and discharge plans; ensure timely documentation
• Supervise assigned assistants and ensure appropriate supervision of support staff
• Maintain accurate and complete clinical records and progress notes

Requirements

• Doctor of Physical Therapy (DPT) degree; licensure to practice physical therapy in the state of California (or eligible with foreign equivalent)
• Active Physical Therapy licensure in state(s) of practice; BLS/CPR certification from an approved vendor
• Valid driver’s license; must be at least 21 years of age
• Knowledge of Home Health Care processes (HCHB and OASIS) preferred; 2 years of recent PT experience required
• Home health experience preferred (1 year); ability to work in a home-based or community-based care setting
• Must meet facility- or program-specific credentialing and safety requirements
