Community Health Navigator & Care Coordinator

Montgomery College

Saint Francis Healthcare System is seeking a Community Health Worker (CHW) / care coordinator to engage patients in preventive and ongoing care, close care gaps, and connect them with community resources to improve outcomes. The role emphasizes addressing barriers and supporting treatment adherence and self-management. Typical hours are full-time, Monday–Friday 8:00am–4:30pm. Requires a high school diploma or GED and BLS certification; CHW certification is preferred. #J-18808-Ljbffr
