Clinical Oversight & Training Manager (LVN) Location: Pomona, CA Job Type: Full-Time Pay: $33.00 – $38.00 per hour Job Summary BLEHEALTH LLC is hiring a Clinical Oversight and Training Manager (LVN) to supervise care teams, ensure quality clinical services, and support care coordination for high-need populations. This role combines clinical leadership, staff training, quality oversight, and community-based care coordination. Key Responsibilities Provide clinical oversight to ECM Care Managers, CHWs, aides, and navigators Review and approve care plans, assessments, and documentation Support medication management, care transitions, and follow-ups Conduct staff training, supervision, and performance monitoring Participate in case conferences and interdisciplinary team meetings Coordinate care with providers, hospitals, and community partners Perform chart reviews, audits, and quality improvement activities Ensure compliance with managed care requirements and regulations Support outreach efforts including home visits and community engagement Qualifications Active California LVN license (required) Minimum 2 years of healthcare management experience (care management or home health preferred) Knowledge of clinical care coordination and quality improvement Strong communication, leadership, and organizational skills Valid driver’s license and reliable transportation required Preferred Experience But Not Required ECM (Enhanced Care Management) programs Medi-Cal or managed care environments Behavioral health or community-based care Working with underserved or high-risk populations Why Join BLEHEALTH?
