Job Details

• Position: Family Nurse Practitioner (FNP)

• Location: Crystal Lake, Illinois

• Schedule: 180 hours over 6 weeks

• Typical Schedule: 2–3 days during the week and every other weekend

• Work Environment: Onsite, patient-facing role

• Start Type: Individual starts

• Additional Requirements: Possible on-call rotation and holiday coverage

Key Responsibilities

• Provide evidence-based primary and family healthcare, including patient assessment, diagnosis, treatment, and management of health conditions.

• Conduct routine physical examinations, health screenings, and diagnostic testing.

• Educate patients on healthy lifestyle choices, preventive care, and chronic disease management.

• Develop and coordinate comprehensive care plans in collaboration with other healthcare professionals.

• Manage patient follow-ups and promote positive health outcomes.

• Support behavioral health, wellness, and chronic condition management.

• Handle administrative duties, including clinic opening and closing, inventory, appointment management, insurance verification, and payment collection.

• Maintain a clean, safe, and professional clinical environment.

• Encourage patients to actively participate in their healthcare and overall well-being.

✅ Required Qualifications

• Minimum of 1 year of recent Nurse Practitioner experience in primary care, family medicine, or urgent care.

• Master’s degree from a Family Nurse Practitioner program.

• Current national board certification as a Family Nurse Practitioner.

• Active NP license in Illinois, in good standing.

• Current BLS certification from the American Heart Association (AHA) or American Red Cross (ARC) .

• Knowledge of chronic disease management and quality measurement.

• Excellent verbal, written, and electronic communication skills.

• Strong organizational, multitasking, and problem-solving abilities.

• Ability to work independently and exercise sound clinical judgment.

• Proficiency with electronic medical records and healthcare technology.

• Willingness to obtain additional state licenses if required.
