This role is a great fit for LVNs who enjoy working independently while still being supported by an experienced care team.What You'll DoProvide direct nursing care in patients' homes according to the plan of careAdminister medications, injections, treatments, and wound care as orderedMonitor vital signs, symptoms, and patient progressCommunicate changes in condition to the RN, physician, and care team as appropriateCoordinate care with the interdisciplinary teamComplete accurate and timely documentation, including EMR documentationFollow infection control, safety, HIPAA, and agency compliance standardsSupport personal care needs within LVN scope of practice when appropriateWhat We're Looking ForCurrent California LVN licenseGraduate of an accredited nursing programCurrent CPR certificationHome health experience preferredWound care and venipuncture experience preferredComfortable with EMR documentationStrong communication, observation, and time-management skillsAbility to work independently and coordinate with a care teamValid California driver's license, auto insurance, and reliable transportationComForCare Home Health Care has been serving whole San Diego community with dedication for 20 years.
