Flex Registered NurseSt. Luke's Health System in Twin Falls, ID is seeking a Flex Registered Nurse to join our Home Care/Home Health team.The Team Support RN performs PRN visits, admission/informational visits, imminent visits and some scheduled visits. Become a part of a dynamic, interdisciplinary team comprised of Case Manager RNs, Rehab Therapists, Respiratory Therapists, Dietitians and aides in the collaborative effort to provide exceptional patient-centered care.Hours: Monday - Friday (8:30am - 5:00pm); may include evening call and rotating weekend coverage.Paid hourly, manageable caseload, pay for drive time, mileage reimbursement, work/life balance!ResponsibilitiesIdentifies patient needs and determines priorities for care through assessment, planning, intervention, and evaluation.Develops, implements and coordinates individualized plan of care for patients assigned through initial and on-going assessments.Initiates assessment based on the patient's problems or needs, the physician's medical diagnosis, and the functional health framework.Performs therapeutic nursing interventions.Manages care of patients.Documents care based on standards of care and practice, patient acuity, and/or clinical prioritiesAdvocates for evidence-based practice care standards and supports adoption of evidence-based practices and innovations in care delivery.Provides patient centric care in alignment with professional nursing practice model.Other duties and responsibilities as assigned.QualificationsEducation: Bachelor's of Nursing DegreeRN's with less than 1 year of RN experience at the time of hire into the RN position must have graduated from an accredited nursing program (ACEN - Accreditation Commission of Education in Nursing, CCNE-Commission of Collegiate Nursing Education)Experience: 0 years' experienceLicenses/Certifications: Current RN licensure from the State of PracticeWhy St. Luke'sAbout Twin FallsLocated in Southern Idaho, just under two hours from Boise, Twin Falls offers four distinct seasons and is best known for Shoshone Falls - "Niagara of the West" - and bountiful outdoor adventures.
