How You'll Make an Impact in This Role

• Oversee and execute personalized care plans to ensure every patient reaches their health goals safely.
• Maintain patient safety through vigilant monitoring and real-time communication of changing conditions to the medical team.
• Act as the central coordinator between all medical disciplines to ensure a seamless and unified treatment experience.
• Lead the discharge planning process to ensure patients and families are fully prepared for success after they leave our care.

What Minimum Requirements You'll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse obtained prior to hire date or job transfer date required.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

What Additional Preferences We're Seeking

• 36 hours a week | Nights, 7 PM - 7:30 AM, every other weekend and rotating holidays
• Prior Acute Care Preferred
