Role Overview FOX Rehabilitation is seeking a compassionate and driven Physical Therapist (Sign on Bonus) to join our clinician‑owned and operated private practice, specializing in geriatric rehabilitation within the Healthcare & Medical Services industry. As a key member of our Healthcare & Medical Services department, you will deliver evidence‑based, patient‑centered care to older adults in their homes and community settings, with a focus on improving functional independence, safety, and overall quality of life. You will perform comprehensive evaluations, develop and implement individualized treatment plans, and use objective outcome measures to guide your clinical decision‑making. Typical interventions include therapeutic exercise, balance and gait training, fall prevention, pain management strategies, functional mobility training, and patient and caregiver education tailored to each client’s goals and living environment. Responsibilities Deliver evidence‑based, patient‑centered care to older adults in their homes and community settings. Perform comprehensive evaluations. Develop and implement individualized treatment plans. Use objective outcome measures to guide clinical decision‑making. Apply therapeutic exercise, balance and gait training, fall prevention, pain management strategies, functional mobility training, and patient and caregiver education tailored to each client’s goals and living environment. Maintain accurate, timely documentation in accordance with professional, regulatory, and organizational standards using electronic medical records and outcome tracking systems. Manage a caseload of older adult patients, prioritizing visits based on clinical need. Demonstrate strong time management and organizational skills. Embod e evidence‑based practice, ethical decision‑making, and compassionate communication, empowering patients and their families through clear education, goal setting, and progress review.
