Key Responsibilities: Provide comprehensive physical therapy evaluations and treatments to patients in a home setting. Maintain a high standard of care , ensuring safety and effective recovery for each patient. Qualifications: Licensed Physical Therapist in the state of Connecticut. Home health or acute care experience preferred. Strong assessment, clinical, and communication skills. Ability to work independently while coordinating care within a multidisciplinary team. Reliable transportation to travel to patient homes within Hartford County .
