We're looking for a Home Health Case Manager who's organized, clinically skilled, and ready to join a supportive team that values autonomy, teamwork, and work-life balance.What You'll Be DoingManage the Care Journey: Serve as the primary point of contact for a caseload of home health patients, ensuring seamless coordination of services.Conduct Comprehensive Assessments: Complete OASIS assessments and develop individualized care plans in collaboration with the interdisciplinary team.Coordinate Services: Work with physicians, nurses, therapists, social workers, and home health aides to ensure timely delivery of care and optimal outcomes.Monitor and Evaluate: Track patient progress, update care plans, and communicate with families and caregivers as needed.Documentation: Maintain accurate and timely clinical documentation in accordance with Medicare guidelines and agency policy.Advocate for the Patient: Provide education, resources, and emotional support to patients and their families to promote independence and wellness.What We're Looking ForLicense: RN (Registered Nurse) with active(state) license.Experience: At least 1 year of experience in home health or case management preferred.Skills: Strong critical thinking, communication, time management, and the ability to work independently.Tech-Savvy: Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or WellSky is a plus).Reliable Transportation: Valid driver's license, auto insurance, and access to a dependable vehicle.Why You'll Love Working With UsSupportive team culture where your input mattersManageable caseloads that respect your time and energyFlexible scheduling so you can balance life and workOpportunities for professional growth and continuing educationA mission-driven environment focused on quality patient careApply Today!
