Responsibilities include: Evaluate residents and develop individualized treatment plans Provide skilled physical therapy services in accordance with physician orders and established care plans Perform therapeutic exercises and functional activities Assist residents with improving strength, balance, coordination, mobility, and endurance Provide gait training and transfer training Recommend appropriate assistive devices and adaptive equipment Monitor resident progress and adjust treatment plans as appropriate Document evaluations, treatments, progress, and discharge information accurately Communicate effectively with nursing, physicians, families, and other members of the interdisciplinary team Participate in care planning and interdisciplinary meetings as appropriate Educate residents, families, and caregivers regarding therapy programs and safe mobility Follow all applicable federal, state, and facility regulations and professional standards Maintain a safe, respectful, and encouraging environment for residents THE IDEAL CANDIDATE We are looking for a Physical Therapist who is: Compassionate and patient-focused Dedicated to improving residents' quality of life Professional and dependable A strong communicator Comfortable working with an interdisciplinary healthcare team Organized and detail-oriented Able to work independently while also being a strong team player Passionate about rehabilitation and helping people reach their goals Experience in skilled nursing, long-term care, rehabilitation, geriatrics, or a similar setting is a plus! QUALIFICATIONS Current Physical Therapist license in the applicable state Graduate of an accredited Physical Therapy program Current CPR certification or ability to obtain Strong clinical and documentation skills Ability to work effectively with residents, families, and healthcare professionals PRN / FLEXIBLE OPPORTUNITIES AVAILABLE We are also open to PRN Physical Therapists!
