Job Opportunity: Physical Therapist

Our organization is in search of a dedicated and empathetic Physical Therapist to become a part of our Healthcare & Medical Services team. In this position, you will deliver exceptional, evidence-based physical therapy services to patients throughout various care settings, including skilled nursing, rehabilitation, home health, and hospice support.

Your responsibilities will include assessing patients with a range of medical and functional requirements, crafting personalized treatment plans, and implementing therapeutic interventions aimed at restoring function, alleviating pain, enhancing mobility, and improving overall quality of life.

• Conduct comprehensive assessments and establish realistic, measurable goals in collaboration with patients, their families, and the interdisciplinary care team.
• Regularly evaluate progress and modify treatment plans as necessary.
• Provide therapeutic exercises, gait and balance training, functional mobility training, and neuromuscular re-education.
• Educate patients and caregivers based on individual abilities and objectives.
• Accurately document all evaluations, treatments, and patient responses in our electronic medical record system, ensuring compliance with regulatory and professional standards.
• Collaborate closely with physicians, nurses, occupational and speech therapists, social workers, and other team members to coordinate patient-centered care plans and facilitate safe discharges or transitions.
• Contribute to fall-prevention strategies, recommend equipment and assistive devices, and assist in home safety planning when necessary.
• Serve as a vital clinical resource by educating patients, families, and caregivers on safe mobility techniques, proper use of assistive devices, energy conservation, and home exercise programs to encourage long-term functional independence.
