Description

• Conduct patient evaluations in inpatient or outpatient environments and develop personalized treatment plans
• Deliver physical therapy interventions that emphasize mobility, strength, stability, and functionality
• Team up with nurses, physicians, case managers, and additional rehabilitation experts to promote optimal results
• Inform patients and families about therapeutic objectives, discharge strategies, and at-home exercise regimens
• Maintain meticulous documentation of evaluations, treatments, and progress, following facility and regulatory protocols
• Engage in care team discussions and interdisciplinary meetings to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (for example, in neurology or geriatrics) are advantageous but not mandatory

Skills

• Prior experience is highly desirable
• Ability to work with patients facing multifaceted medical and mobility challenges
