Registered Nurse Case Manager

Job Summary:

The Registered Nurse - Case Manager provides skilled nursing care and care coordination within an acute care facility. This role involves case management, discharge planning, and collaboration with interdisciplinary teams to ensure effective patient care. The nurse will perform patient assessments, manage care plans, and support patients through the discharge process, requiring at least 2 years of acute medical care management experience. Strong communication and organizational skills are essential to succeed in a fast-paced hospital environment. Responsibilities:

• Perform care coordination and discharge planning for acute medical patients.
• Conduct patient assessments and develop individualized care plans.
• Collaborate with interdisciplinary teams including physicians, social workers, and other healthcare providers.
• Manage patient ratios with a typical load of 1:25 during day shifts.
• Provide case management to ensure continuity of care and appropriate resource utilization.
• Maintain accurate documentation and patient records in compliance with hospital policies.
• Support patients and families through education and communication regarding care plans and discharge instructions.

Required Skills & Certifications:

• Active Registered Nurse license in California; pending licenses accepted but must be licensed upon start.
• Minimum 2 years of acute medical care management experience performing care coordination and discharge planning.
• Strong clinical assessment and patient management skills.
• Excellent communication and interpersonal abilities.
• Ability to manage a patient ratio of approximately 1:25.
• Capability to work effectively in a fast-paced acute care environment.

Preferred Skills & Certifications:

• Experience with case management in acute care settings.
• Familiarity with hospital discharge processes and resource coordination.

Special Considerations:

• No floating expectation for this role.
• First-time travelers must pass screening prior to assignment.
• Typical weekend requirement is one full weekend per four weeks, subject to departmental needs.
• Dress code includes business attire or scrubs.
• Return to office (RTO) restrictions do not exceed 7 days.
• Cancellations due to schedule changes may result in a systemwide Do Not Return (DNR) status.

Scheduling:

• Day shifts with patient ratios of 1:25.
• Work schedule includes typically one full weekend every four weeks, based on department requirements.
• Start date is ASAP with a defined winter increase period from October 4, 2026, through February 27, 2027.
