Hospice Field Admissions RN — Shape Compassionate Home Care
MJHS
MJHS is seeking a Hospice Field Admission Nurse (RN) to perform initial physical and psychosocial assessments for potential hospice patients in private homes, hospitals, or nursing facilities. You will develop individualized plans of care and goals and make recommendations based on patient needs. The role includes admitting eligible patients to the Hospice program following NHPCO standards. A NYS RN license and NYS Driver’s license are required, with BSN preferred and 2+ years in #J-18808-Ljbffr MJHS
