Benefits may vary by employment status Responsibilities Provide 1:1 care for clients following the client's plan of care at all times while maintaining their dignity and independence. Administer medications, feedings, oxygen, and ostomy care as needed per the client's plan of care and assessment results. Accurately document nursing actions and communicate with family, case manager, and physicians, documenting progress and outcomes for established goals and informing all parties of changes in the client's medical condition and needs. Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are always on hand. Ensure all patient needs are met in accordance with the physician's instructions. Qualifications LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and residence. Valid driver’s license, acceptable driving record, and proof of car insurance. CPR professional certification. 1+ years of nursing experience preferred. Home health care experience preferred.
