Registered Nurse (RN) - Hospice Palliative Care Lead

As a Registered Nurse (RN) - Hospice Palliative Care Lead, you will play a vital role in our organization's mission to provide exceptional care and comfort to our patients and their families. Located in Toledo, Ohio, our state-of-the-art facility is designed to provide a warm and welcoming environment for our patients to recover and heal.
About the Role:
Lead a team of Hospice Palliative Care RNs in providing compassionate and high-quality care to patients with life-limiting illnesses
Collaborate with interdisciplinary teams to develop and implement personalized care plans that focus on symptom management, pain control, and improvement of quality of life
Provide expert nursing assessment, intervention, and education to patients and their families
Manage care transitions and ensure continuity of care across settings
Conduct initial and ongoing assessments, identifying and addressing physical, emotional, and spiritual needs
Responsibilities:
Develop and maintain personalized care plans that meet the unique needs of each patient
Collaborate with interdisciplinary teams to develop and implement care plans
Provide expert nursing care and guidance to patients and families
Maintain accurate and timely records of patient care
Participate in quality improvement initiatives to improve patient outcomes and satisfaction
Act as a clinical resource and mentor to junior staff members
Requirements:
Current RN license in the state of Ohio
Bachelor's degree in Nursing (BSN) or related field
Minimum 2 years of experience in Hospice Palliative Care
Knowledge of current nursing standards, practices, and regulations
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
