RN - Case Manager

Registered Nurse (RN) - Case ManagementWe are seeking a dedicated and experienced Registered Nurse to coordinate management of patient care, ensuring optimal resource utilization, service delivery, and compliance with regulatory requirements. The ideal candidate will provide comprehensive support through assessment, care planning, plan implementation, and evaluation of individual patient needs.Responsibilities:Coordinate management of care and optimize resource utilization.Deliver ongoing support and expertise through comprehensive assessment and care planning.Implement care plans and evaluate individual patient needs while respecting patient preferences.Enhance the quality of patient management and satisfaction to promote continuity of care and cost-effectiveness.Integrate functions of case management, utilization review, and discharge planning.Advocate for patients by addressing holistic needs and facilitating resource utilization.Qualifications:Active Registered Nurse (RN) license required.Must possess BLS certification through the American Heart Association.Minimum of 2 years of recent experience in RN Case Management.Experience in Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) settings is preferred.Familiarity with care coordination, discharge planning, and Interdisciplinary Team (IDT) collaboration.Work Schedule:This position will follow a regular Monday to Friday schedule, with working hours from 8:00 AM to 4:30 PM.Join us in making a difference in patient care through dedicated management and coordination efforts!
