The Doe Fund in Brooklyn, NY is seeking a high-energy, self-motivated Case Manager based at the Porter Avenue Facility. You will manage a caseload of clients, coordinate referrals for mental health and housing services, and collaborate with DHS and other agencies to support program goals.
