Licensed Practical Nurse (LPN) – Travel, Home Healthcare, Lynchburg, Virginia. Start date 09/14/2026 for an 8-week assignment. Shift: 8-hour days. Guaranteed hours: 40 per week. Monitor patient status, document clinical findings, and report changes to the assigned RN Case Manager; participate in care planning and coordination with the interdisciplinary team. Communicate effectively with patients, families, and colleagues; ensure clear documentation of outcomes and interventions in the medical record. Educate patients and families on appropriate self-care techniques; assess learning progress and adjust teaching as needed. Apply standard infection control practices and safety protocols; assist with end-of-life care duties as appropriate and support admission, discharge, and transfer activities; may participate in on-call duties. Requirements Active LPN/LVN license in Virginia; current CPR certification. Proof of Hepatitis consent/declination and TB vaccination record. Minimum 2 years of LPN/LVN experience in a clinical setting; home health or hospice experience preferred. Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing. Valid driver’s license with current auto liability insurance; reliable transportation required. Proficiency with electronic medical charting and time-keeping software (Homecare Homebase) and ability to document accurately; fluent English and reliable attendance.
