Licensed Practical Nurse (LPN) - Full Time, Weekend Shift
Schedule: Full Time - Day/Night Variable (Every Weekend, 40 hours per week)
Assignment Duration: This is a 13-week contract position with the potential for extension based on performance and need.
Job Description: The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for residents, ensuring that care is appropriate to the age-specific needs of the adult and geriatric patient populations. The LPN will work under the supervision of a Registered Nurse (RN) and maintain collaborative relationships with other nursing staff, physicians, and ancillary staff.
Responsibilities:
- Collect data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and Interdisciplinary Team.
- Contribute to creating developmentally appropriate and culturally relevant care plans, reflecting nursing diagnoses and interventions.
- Evaluate resident outcomes and recommend revisions to care plans as necessary.
- Collaborate with residents/families and healthcare teams to ensure comprehensive care and customer satisfaction.
- Maintain current knowledge and competency in nursing practice, participating in mentoring and orientation of staff.
- Provide leadership in the professional practice setting under the direction of the RN.
- Ensure safety and quality in care while considering cost-effectiveness.
Required Experience / Certifications / Licensure:
- Current NY LPN Licensure
- Basic Life Support (BLS) certification
- Minimum of 1 year of experience serving in an LPN capacity, preferably in a Long-Term Care (LTC) facility
