Overview CareOne at Weymouth, in partnership with BridgeCare Provider Group, is hiring an Internal Medicine Nurse Practitioner. Salary range: $125,000 to $135,000 annually. Compensation based on experience, qualifications, credentials and other relevant information. Responsibilities Conduct patient visits in accordance with local, state, and federal regulations. Communicate visit findings to designated members of the interdisciplinary team. Facilitate access to appropriate support models of care (e.g., rehabilitation, ongoing aggressive treatment). Consult with other members of an interdisciplinary team to contribute to the patient’s plan of care. Prepare and maintain accurate patient records, charts, and documents. Assist in development of clinical practice guidelines/standards in support of quality care as needed. Ensure collaboration with patients, caregivers, medical directors, and members of IDT. Education Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program. Professional Experience Must hold a Nurse Practitioner’s license in the state of operations. 1+ year experience working within Family, General Practice, Geriatric care or a related field. Valid driver’s license and reliable transportation. Willingness to travel locally. Experience in a nursing home, hospital, assisted living or acute rehab space is a plus but not required.
