This role provides patient centric care and is primarily responsible for nursing assessment and diagnosis, planning, implementation, and evaluation.This position will be 0.75FTE, three 10 hour shifts from 7:30am-6pm.ResponsibilitiesAssesses physical, emotional, social, and spiritual needs and evaluates patients' adaptation to health changesDevelops a plan of care based on patients' disease, symptoms, and response to treatmentEducates patients, families, and caregivers on disease processes, treatment, side effects and symptom managementCoordinates with inter-disciplinary team to ensure continuity of careRefers patients, families and caregivers as appropriate to other clinical resources/professionals such as social work, home health care, clinical nutrition, etc.Implements, evaluates and modifies plan of careIntegrates research and evidence based knowledge into clinical practiceIn addition to primary responsibilities of the Registered Nurses (RN), the Clinical Nurse Coordinator (CNC) will have the following key responsibilities:Direct Patient CareEducation patients and families: chemotherapy, central lines, injections, home careTelephone and email symptom triageDressing changes, medication administration, injectionsNew patient orientationCaregiver supportAssessment of needs for additional supportive care resourcesPrescription refillsResult reviewsIndirect Patient CareSupportive care referralsFMLA paperwork, disability paperwork, Ferry passesCoordination of outside clinical careEnsuring continuity of care with outside providersCollaboration with research team for patients on trialCommunication and TeamworkAttend regular team meetings with provider and nursing teamsCoordination of care with ancillary servicesDelegation of tasks to appropriate team membersCoordinating patient schedules with clinic teamCollaboration with infusion RN and other members of the care team to assure patient needs are metReal-time management and escalation of issues in clinicProfessional Development and Clinic EngagementDocument all nursing care activities accurately and in a timely fashion.Demonstrate knowledge and understanding of patient privacy rights.Active participation in continuing education, departmental projects, committee work, and quality improvement initiativesQualificationsRequired:Graduate from an accredited school of nursingCurrent Washington State Registered Nurse (RN) license or NLC multistate license.Current BLS AHA Health Provider Card (or equivalent course, such as by the American Red Cross) and renewal required every two yearsAdditional certification may be required dependent upon department.Critical thinking; the ability to practice the cognitive skills of analyzing, applying standards, discriminating, information seeking, logical reasoning, predicting, and transforming knowledgeAbility to develop clinical judgment.Time Management skills; the ability to organize and manage time and tasks independentlyAbility to communicate effectively by looking and listening for cues, asking open-ended questions, exploring cues, using pauses, screening responses, and clarifying response.Ability to effectively listen by using reflecting, acknowledging, summarizing, empathizing, and paraphrasing skill.Ability to effectively provide information by checking what information the person knows already, giving small amounts of information at a time, using clear terms and avoiding jargon, avoiding detail unless it is requested, checking for understanding, and pausing and waiting for a response.Preferred:BSN prepared nurseOCN (Oncology Certified Nurse) or BMT RN2 years of oncology nursing experiencePrevious telephone triage experiencePrevious outpatient clinic experienceTakes initiativeGood communication skillsSkilled at multitasking and organizationThe hourly pay range for this position is from $45.35 to $84.80 and pay offered will be based on experience and qualifications.
