We are seeking a compassionate and dedicated Full-Time Family Nurse Practitioner to join our dynamic team at Elmira Wellness Service, located in Queens Village, New York. In this crucial role, you will diagnose and treat a variety of health conditions while actively promoting preventative healthcare practices. As a highly skilled advanced practice registered nurse, you will play a vital role in delivering the highest standard of patient care within our home care setting.

Our interdisciplinary team comprises five physicians and five nurse practitioners, all focused on enhancing the overall well-being of our patients. Work Environment

This position offers a full-time schedule of 40 hours a week, primarily working from Monday through Friday, starting at 5:00 pm. Our home care setting allows you to work closely with patients in their own environments, fostering relationships that can lead to improved health outcomes. Key Responsibilities

• Conduct thorough physical examinations and document patients’ medical histories.
• Order and interpret diagnostic tests (such as blood tests and X-rays) to evaluate patients' health status.
• Diagnose a range of health conditions and illnesses, determining appropriate treatment plans.
• Initiate and oversee treatment for chronic and infectious diseases, adapting as necessary based on patient response.
• Prescribe medications and manage patient medication regimens to ensure adherence and effectiveness.
• Maintain accurate and timely health records, using electronic health record systems for efficiency.
• Educate patients and their families on health conditions, treatment options, and preventive measures.
• Promote healthy lifestyle choices, guiding patients in disease prevention techniques.
• Advance your professional expertise through ongoing research and participation in educational seminars.

Patient Population / Client Focus

In this role, you will primarily serve a diverse patient population within Queens Village, addressing their unique health needs through tailored care. Our patients range from those with chronic illnesses requiring ongoing management to families seeking guidance on preventive healthcare practices. Your contributions will directly impact our patients’ quality of life and overall health outcomes.

Qualifications

• A Master’s degree in Nursing with a specialization as a Nurse Practitioner.
• A valid New York State Nurse Practitioner license (ANP/FNP required).
• Possession of a DEA number to prescribe medications.
• Proven experience as a Nurse Practitioner in clinical, hospital, urgent care, or similar settings.
• Sound knowledge of health regulations and patient care best practices.
• Excellent communication and interpersonal skills to effectively engage with patients and team members.
• A strong ethical framework, demonstrating integrity and professionalism at all times.
• A positive attitude and an ability to work collaboratively within a team-oriented environment.

Preferred Experience

While all candidates are encouraged to apply, preference will be given to those with clinical experience in family health, internal medicine, cardiology, or urgent care settings. Bilingual candidates are also highly desirable as we aim to serve our diverse community effectively.

Compensation and Benefits

This full-time Nurse Practitioner position offers a competitive annual salary ranging from USD 140,000 to 180,000, commensurate with experience. In addition, we provide an extensive benefits package that includes:
