We are currently seeking a compassionate and reliable Transitional Aide to join our team.

Requirements:

• MUST have FULL flexible availability (days, evenings, weekends as needed)

• Reliable transportation

• Ability to assist clients during transitional care periods

• Strong communication and time-management skills

• Dependable and professional demeanor

Responsibilities:

• Provide support and assistance to clients during care transitions

• Assist with daily living activities as needed

• Follow care plans and agency policies

• Communicate effectively with supervisors and care team
