The Patient Care Coordinator functions as an integral member of the team and is the first point of contact for all people inquiring about the provider's practice. The primary role is to organize the practice's daily activities and paperwork. Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service and adhere to Lean processes. Primary Responsibilities:- Exhibits empathy, courtesy, competence, efficiency, and care
- Schedules appointments and procedures, following standard guidelines
- Uses multiple computer and phone systems to fulfill patient needs over the phone
- Verifies patient information, while documenting in their Electronic Health Record (EPIC)
- Advises patients of their responsibilities regarding insurance and referral information
- Greet customers into the practice and provide instruction and/or direction as necessary on the phone
- Schedule and coordinate all patient appointments as directed
- Prepare all documentation and records with accuracy as directed or necessary for the patient's care
- Meet or exceed the performance measures established by the Practice Management Department
- Follow establish policy and procedures related to the position
- Participate and contribute to a team-based environment
- Maintain a team supportive attendance record
- Understand and deliver quality customer service that is consistent with the Polyclinic's efforts in becoming a High-Performance Organization
- Follow OSHA/WISHA (including usage of PPE) guidelines
- Follow HIPAA regulations
- Adapt and perform effectively in a continuously changing environment, and in occasional difficult situations
- P erforms other duties as assigned

- High School Diploma/GED
- One plus years of experience computer proficiency (multi-task through multiple applications including Microsoft Outlook, Excel, and Word)
- One plus years of experience working in a medical office position performing duties such as scheduling appointments, checking patients in/out, insurance verification, collecting co-pays and balances due, and maintaining medical records
- Must reside in a location near Everett, WA as orientation is onsite
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
- Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
- Experience taking inbound and outbound calls
- One plus years of experience working knowledge of Epic EMR software
- Ability to work independently and maintain good judgment and accountability
- Demonstrated ability to work well with health care providers
- Strong organizational and time management skills
- Ability to multi-task and prioritize tasks to meet all deadlines
- Ability to work well under pressure in a fast-paced environment
- Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others
