Job Summary

ALIYA of 87th is seeking an experienced Wound Care Nurse – RN/LPN to oversee the assessment, treatment, and ongoing management of residents with wounds and skin integrity concerns. The Wound Care Nurse will work closely with nursing staff, physicians, residents, and the interdisciplinary team to develop and implement effective wound care plans and promote optimal healing outcomes.

Duties & Responsibilities

• Assess, monitor, and document residents’ wounds, skin conditions, and risk factors for skin breakdown.

• Develop and implement individualized wound care treatment plans in accordance with physician orders.

• Perform wound treatments and dressing changes according to established protocols.

• Monitor wounds for healing, infection, deterioration, and other complications.

• Communicate changes in wound status to physicians, the Director of Nursing, and appropriate healthcare providers.

• Collaborate with nursing staff and the interdisciplinary team to ensure effective wound management.

• Provide education and guidance to nursing staff regarding wound prevention, treatment, pressure relief, positioning, and skin care.

• Monitor residents at risk for pressure injuries and ensure appropriate preventive interventions are implemented.

• Maintain accurate and timely wound assessments, treatment records, and required documentation.

• Participate in care plan meetings and provide updates regarding residents’ wound status and treatment plans.

• Monitor the effectiveness of wound care interventions and recommend changes when appropriate.

• Coordinate with outside wound care providers and specialists as needed.

• Ensure wound care practices comply with facility policies and applicable regulatory requirements.

• Assist with quality improvement initiatives related to wound care and skin integrity.

• Perform other duties as assigned by the Director of Nursing.

Qualifications

• Current Illinois RN or LPN license in good standing.

• Previous wound care experience preferred.

• Skilled nursing, long-term care, or post-acute experience preferred.

• Wound Care Certification (WCC, CWCN, CWS, or equivalent) preferred.

• Strong knowledge of wound assessment, prevention, treatment, and documentation.

• Strong clinical assessment, critical thinking, and problem-solving skills.

• Excellent communication and interpersonal skills.

• Ability to educate and collaborate effectively with nursing staff and interdisciplinary team members.

• Strong organizational skills and attention to detail.

• Experience with electronic medical records; PointClickCare experience preferred.
