Key Responsibilities Provide patient education on cardiovascular health, lifestyle modifications, medication management, and preventive measures to promote heart health and reduce cardiovascular risk. Maintain accurate and timely documentation of patient encounters, diagnostic findings, treatment plans, and follow-up recommendations in the electronic medical record system. Conduct comprehensive assessments, including history-taking, physical examinations, and diagnostic evaluations. Diagnose and treat a wide range of conditions. Develop and implement individualized treatment plans based on patient needs, preferences, and medical history. Collaborate closely with healthcare team to ensure coordinated and comprehensive patient care. Document patient encounters, treatment plans, and follow-up recommendations accurately and timely in the electronic health record system.
