Case Manager

We are seeking a qualified and experienced Case Manager to join our team. The ideal candidate will possess a strong background in acute care case management, particularly within California, and will be committed to providing excellent patient care.

Job Requirements:

Experience: Minimum of 2 years of acute care case manager experience, with previous experience in California.
Licensure: Active California Registered Nurse (RN) License.
Certifications: Basic Life Support (BLS) certification required.
Flu Vaccine: Flu vaccination is required with no exemptions. Flu season begins in September.
Special Requirements: Experience with EPIC is highly preferred. Must have California case manager experience (6 months is acceptable).

Floating Requirements: No.
On-Call Requirements: No.
Pending License: Must have an active license at the time of submission.
Scheduling Requests: No scheduling requests can be accommodated.

Shift Details:

Shift: Day shift, 5 days a week, 8 hours per day (08:00 - 17:00).
Block Schedules: Not applicable.
