Position Summary

The Swing Bed Coordinator & Marketing Specialist is responsible for coordinating and managing the facility’s Swing Bed program while supporting census growth through relationship development, community outreach, referral marketing, and communication with hospitals, physicians, case managers, and other healthcare partners.

This position serves as a key liaison between the facility, patients and families, referral sources, and the community. The coordinator ensures that Swing Bed referrals are processed efficiently, patients receive appropriate follow-up, required documentation is completed, and marketing efforts effectively promote the facility’s services.

Essential Duties and Responsibilities

Swing Bed Coordination

• Coordinate the admission and referral process for Swing Bed patients from initial referral through admission and discharge.
• Serve as the primary point of contact for hospitals, discharge planners, case managers, physicians, patients, and families regarding Swing Bed referrals.
• Review referrals and gather required clinical, demographic, insurance, and other admission information.
• Coordinate with nursing, therapy, social services, dietary, billing, and other departments to facilitate timely admissions.
• Monitor referral status and follow up promptly on pending referrals and potential admissions.
• Assist with insurance verification, prior authorization, and other payer requirements as applicable.
• Maintain accurate and timely documentation related to referrals, admissions, and patient transitions.
• Track Swing Bed census, referrals, admissions, discharges, and referral outcomes.
• Identify barriers to admission and communicate them to appropriate members of the care team.
• Participate in discharge planning and assist with coordination of services needed after discharge.
• Maintain compliance with applicable federal and state regulations, facility policies, and Swing Bed program requirements.
• Protect patient confidentiality and follow HIPAA requirements at all times.

Marketing and Business Development

• Develop and maintain professional relationships with hospitals, physicians, physician offices, discharge planners, case managers, social workers, home health agencies, hospice providers, assisted living communities, and other referral sources.
• Conduct regular outreach to existing and prospective referral partners.
• Represent the facility professionally at community events, healthcare networking events, meetings, and educational programs.
• Schedule and conduct facility tours for prospective patients, families, referral sources, and community partners.
• Communicate the facility’s Swing Bed, rehabilitation, nursing, and other applicable services to referral sources and the community.
• Develop and distribute marketing materials, presentations, newsletters, and other promotional materials as appropriate.
• Assist with social media, website content, community communications, and other marketing initiatives.
• Maintain a current database of referral sources and document outreach activities.
• Monitor referral trends and identify opportunities to increase awareness and utilization of the Swing Bed program.
• Work with leadership to develop and implement census-building and community outreach strategies.
• Gather feedback from referral sources and identify opportunities to improve the referral and admission experience.
• Maintain a professional presence and positive image for the facility throughout the community.

Communication and Collaboration

• Maintain open communication with facility leadership and interdisciplinary team members.
• Provide regular updates regarding referral activity, potential admissions, census, and marketing efforts.
• Collaborate with clinical and administrative departments to ensure a smooth patient experience.
• Respond promptly and professionally to inquiries from referral sources, patients, families, and community partners.
• Escalate concerns or barriers appropriately to facility leadership.

Qualifications

• High school diploma or equivalent required.
• Associate or bachelor’s degree in healthcare administration, marketing, business, nursing, social work, or a related field preferred.
• Previous experience in healthcare, hospital discharge planning, skilled nursing, Swing Bed, rehabilitation, admissions, referral coordination, or healthcare marketing preferred.
• Strong knowledge of healthcare referral and admission processes preferred.
• Excellent communication, organizational, interpersonal, and customer service skills.
• Ability to establish and maintain strong professional relationships.
• Strong attention to detail and ability to manage multiple priorities.
• Comfortable working independently while collaborating effectively with an interdisciplinary team.
• Proficiency with Microsoft Office and electronic health records or referral-management systems preferred.
• Ability to maintain confidentiality and comply with HIPAA requirements.

• Ability to work in a healthcare environment and interact with patients, families, healthcare professionals, and members of the public.
• Ability to use a computer, telephone, and other standard office equipment.
• May occasionally require evening or weekend participation in community events or facility activities.
