Physical Therapist (PT) – Home Health | Ocean County & Barnegat, NJ | PRN Stern at Home Therapy is hiring a PRN Physical Therapist (PT) to provide one-on-one care to patients throughout Ocean County, NJ, including the Barnegat area . This opportunity is ideal for a Physical Therapist looking to supplement an existing schedule, maintain a small caseload, or add flexible home health work while maintaining control over when and where they see patients. With a minimum commitment of just 5 patients , our PRN option provides flexibility while allowing therapists to build a geographically concentrated caseload within their preferred areas of Ocean County. What You'll Do: As a Physical Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes. You'll maintain clinical autonomy over evaluations and treatment plans, while our administrative team supports referrals, scheduling, authorizations, and patient coordination. Responsibilities include: Perform physical therapy evaluations and develop individualized plans of care. Provide one-on-one physical therapy treatments in patients’ homes. Help patients improve strength, balance, mobility, endurance, and functional independence. Provide gait training, transfer training, therapeutic exercise, and fall-prevention strategies. Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate. Educate patients and caregivers on exercises, mobility strategies, and safety. Monitor patient progress and modify treatment plans as clinically appropriate. Complete accurate and timely clinical documentation. Communicate with physicians, caregivers, families, and interdisciplinary care team members. Compensation: PRN 1099 – Minimum 5 Patients: $72 per follow-up visit. $85 per evaluation. Fixed per-visit compensation with no tiered volume model. PRN positions are 1099 independent contractor roles only. Why Join Stern? Minimum commitment of just 5 patients.
