NORC Nurse -Per Diem Manhattan

Overview Works as a member of an interdisciplinary care team at a NORC (Naturally Occurring Retirement Community) or Senior Living Retirement Community location and is responsible for coordinating a broad range of health and social services to help support older residents to age in their own homes. NORCs facilitate and integrate the health and social services already available in the community while organizing additional services and supports necessary to help meet the goal of enabling older adults to remain in their community. Internal mobility and advancement opportunities. What You Will Do Collaborate with site staff to ensure residents engage in self‑care management health strategies. Provide health‑focused outreach to seniors who may benefit from supportive services provided by the program. Promote and encourage healthcare activities such as health screening, assessments, health care linkages, and health presentations. Make telephone calls and home visits to individual residents aimed at assessing their needs and supporting successful community living activities. Make referrals to appropriate health services based on the assessed needs of the resident. Provide Health Care Advocacy by acting as a liaison between various healthcare systems during times of transition. Assist residents needing direct patient care to connect with their physician or other appropriate health care services to address their individual needs. Document all nursing interventions provided as required by site location. Participate in special projects and perform other duties as assigned. Qualifications License and current registration to practice as a Registered Professional Nurse in New York State. CPR/BLS Certification. Associate’s Degree in nursing (required) and Bachelor’s Degree in nursing (preferred). Minimum of two years of experience working as an RN in LHCSA or CHHA. Prior experience working with an aging population and providing clinical assessments of their health needs. Pay Range USD $40.95 – USD $51.19 per hour. #J-18808-Ljbffr
