It is usually assigned to a Medical-Surgical or Telemetry unit and works alongside a Social Worker. The candidate must be able to operate independently on the unit while collaborating with an interdisciplinary team and patients for discharge planning. Duties include rounding with physicians and the care team. A positive attitude, critical thinking, and effective time management are essential. Utilization Review (UR) is managed by a separate department responsible for insurance approvals and transportation arrangements. Caseload Typical floors have 33-40 patients. Most floors have 1 Case Manager (CM) and 1 Social Worker (SW); caseload ranges from 17-25 patients. Pilot floors have 1 CM and 2 SWs; caseload ranges from 12-17 patients. Must be able to manage at least 20 patients. Assignments are generally on Med-Surg or Telemetry floors following Internal Medicine and Family Medicine. Electronic Medical Record (EMR) Allscripts Preferred Experience and Knowledge Familiarity with CarePort, MCG, and/or InterQual criteria is preferred. Some experience in Utilization Review is preferred. Position Focus Discharge planning Coverage Area Inpatient Common Patient Types and Duties Discharge planning to home, community, or skilled nursing facilities (SNF). Assisting with housing and shelter packets for patients experiencing homelessness. Coordinating hemodialysis and wound care referrals (e.g., KCI for wound vacs). A resource binder is provided. Experience Requirements Minimum of 1 year inpatient case management experience required. Critical care experience is preferred. Supervision Case Management Manager Shifts and Scheduling Monday through Friday, 8:00 AM to 4:30 PM (7.5-hour shifts with a 1-hour lunch). Breaks follow facility policy; management supports taking a full lunch break. Weekend and Holiday Requirements Occasional weekend shifts, which are rare and require manager approval. Weekend caseloads differ and may require coverage of multiple units. Occasional holiday shifts, which are rare and require manager approval. Dress Code Business casual or any color scrubs except royal blue or maroon (neutral colors preferred). Lab coat provided. Additional Notes Respiratory Therapy is not involved in discharge planning. Physicians handle medication reconciliation and send orders to pharmacy.
