Position Overview The LPN is responsible for providing quality patient care and performing skilled care in the patient’s home. Responsibilities Provide services within the scope of practice as defined by state laws and in accordance with the plan of care. Collaborate with physicians and RNs, documenting care accurately and promptly, and may not alter the plan of care. Instruct the patient’s family in the care of the patient and maintain a healthy environment. Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in condition daily. Qualifications One year of clinical LPN experience. Current LPN license in the state where services are rendered. Current CPR certification. Driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation. Reliable commute to multiple patient residences daily within the assigned geographic area.
