The Patient Care Coordinator/ Biologic Coordinator I serves as the primary "patient navigator" for individuals receiving biologic and specialty therapies through the pharmacy. This role focuses on ensuring patients remain on therapy through proactive refill coordination, documentation management, and ongoing patient engagement. The Biologic Coordinator I works closely with pharmacy, nursing, operations, and sales teams to ensure timely medication access and continuity of care. This role is highly patient-focused and responsible for building strong relationships with assigned patients while supporting operational workflows necessary for safe and timely therapy delivery.

Key Responsibilities

Patient Navigation & Relationship Management

• Serve as the primary point of contact for assigned biologic therapy patients.
• Develop and maintain ongoing relationships with patients to support therapy adherence and engagement.
• Conduct refill assessments and coordinate medication refills or infusion scheduling.
• Provide general therapy education within scope and regulatory parameters and reinforce pharmacy support resources.

Therapy Coordination

• Coordinate medication deliveries and infusion scheduling with patients and internal teams.
• Monitor therapy schedules to ensure refills and infusions occur on time.
• Track upcoming therapy needs and proactively contact patients prior to refill or infusion dates.

Documentation & Record Maintenance

• Ensure required patient documentation is current, including signed orders, clinical notes, insurance authorizations, and therapy documentation.
• Maintain accurate patient records and progress notes in the pharmacy system.

Care Coordination

• Communicate with providers, intake teams, nurses, and pharmacists to obtain missing documentation or resolve therapy barriers.
• Identify and escalate patient concerns, therapy delays, or adherence barriers to the appropriate clinical team.

Patient Support

• Address patient questions related to scheduling, medication delivery, and general therapy processes.
• Reinforce adherence to prescribed therapy schedules.
• Provide timely follow-up on outstanding patient needs.

Education:

• High school diploma or equivalent required

Certifications (Preferred but not required):

• Certified Patient Navigator (CPN)
• Case Management Certification (CCM or ACM)
• Certified Pharmacy Technician (CPhT)

Experience:

• Proficiency in maintaining medical documentation and working within HIPAA-compliant systems.
• Healthcare

Preferred Experience (but not required):

• 2–4 years of experience in patient coordination, case management, or healthcare navigation, preferably involving biologic or specialty medications.
• Familiarity with specialty pharmacy operations

• Licensed Practical Nurse
• Registered Nurse
• Healthcare administrative experience

Must hold valid and active licensure or certification in one of the following:

• Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) must be licensed in the state of employment.
• Licensed Pharmacy Technician or Certified Pharmacy Technician (CPhT) certification from PTCB or NHA and registration with the state board of pharmacy, if applicable.
